The above requirements should be submitted on or before December 24, 2011. Releasing of gifts will be from Sunday, December 25, 2011 between 7:00 AM to 8:00AM only until December 31, 2011.1. Original copy of your Birth Certificate. (NSO authenticated)
2. Original copy of your parent's Marriage Certificate. (NSO authenticated)
3. Picture of you with me on your baptism.
4. Baptismal Certificate with my name on it. (with official seal from the Parish)
5. Affidavit of at least 3 disinterested persons, duly notarized.
6. Barangay Clearance.
7. Police and NBI Clearance.
